If there is a blinking box, you don't have Adobe Acrobat, and you must install, by clicking the Adobe button at top of page. |

If you try to fill out form, and you are unable to type.. you probably have an old version of Adobe Acrobat. It must be 4.0 or later. So uninstall the old version., then install the newer version, by clicking on button at top of page. |
| 3. | To fill out form, click on the "hand" on the Adobe Acrobat toolbar (right above the form), then click on the line where you want to type, then begin typing. To make a "Check" in a box, just click inside the desired box. |
| 4. | To print: Make sure you are pressing the print icon through Adobe Acrobat.(Directly above the form). Make sure that you are not trying to print through you browser. |
