rage volunteer reimbursement form

Pleasanton Girls Soccer is greatly dependent on the willingness of our membership to dedicate some time to many jobs, which include coaching our daughters, becoming a referee, making sure the fields are left the way we found them, assisting at various club functions like registration and at the Parade, helping with mailings, serving on the Board or committees of the Board, and many other duties which must be performed to enhance the experience of about 2000 young ladies. In return for your efforts, the club would like to "thank you" by sending you $50 or providing you with a letter which you may use when filing your taxes, that you have made a $50 contribution to this non-profit organization.

Volunteer Reimbursement Form

This form is used for reimbursement or tax donation of the $50 Volunteer Fee. Send this form after performing an approved volunteer activity (see examples listed at bottom).
Your service must be completed or coach assignments must be made before requesting the $50 "thank you" payment.
Note: individual team responsibilities (such as publicity, treasurer, snacks, etc.) do not qualify for reimbursement.

Deadline for reimbursement is December 1.

NOTE: Please be aware that helping at the summer tournaments is expected of every Competitive Teams family. We appreciate your help, but this is not a job for which the club will make a $50 reimbursement. Instead each team has its application fee waived. Additionally many teams, Rec and Comp, have job requirements, such as treasurer, in charge of shade, etc. Those, too, are team-specific, which help your daughter's team. The time and effort may be significant and essential, but our financial "thank you" is meant for coaches who volunteer many, many hours of their free time, or support the entire effort of your club and are not team-specific.
